Ordering Telecommunications Services

All requests for telecommunications and data network services, including installation, moves or disconnection, must be submitted via the Telecommunications Services Request Form.

For telephone installations and moves, please include a room layout with the desired location of telecommunications equipment.

Under Ray Baum's Act, telephone users are required to report their location to ensure E911 (Enhanced 911) dispatchers know where callers are. Report all phone moves via the Telecommunications Services Request Form.
