Requesting Telephone Disconnects

Requests for disconnecting telephone equipment must be submitted by email to AngelaCockrum@missouristate.edu. The following information is required:
1. Telephone number to be disconnected and disconnect date.
2. Budget to be charged and CC of individual authorized to expend department funds.
3. Include requester contact information. The system administrator will need this information to complete the request.
